Physical Examinations

We encourage all patients to come in for annual exams to insure early detection of illness or disease. Detecting disease early can save your life or prevent disability and severe suffering. Types of annual exams may include pap smears, breast exams, rectal exams, prostate exams, and school or work physicals.

Well Child Exams

We offer exams to check the growth and development of your child. This is also the time when parents may discuss with the provider any concerns regarding the development of their child. The provider will discuss health promotion activities and offer guidance about developmental milestones, safety issues, and immunizations.

Pre-Natal Care

We offer the initial prenatal exam and then refer patients to competent facilities for the rest of their prenatal care.

Diabetic Care

Diabetic patients will have appointments on a regularly scheduled basis depending on their medical condition in order to optimize the management of their diabetes and minimize the long-term complications of the disease. Laboratory work will be conducted to evaluate the effectiveness of the current therapy. Adjustments in care and treatment are done in collaboration with the provider and the patient.

Home Visits

If the provider determines a patient needs home visits, a Community Health staff member will go to the patient's home and provide nursing services. This may include, dressing changes and wound assessment, education or help with a new medication, home safety assessments, immunizations, follow-up on abnormal lab results, and blood pressure checks.

Laboratory Services

We offer a limited number of in-house laboratory testing services. For most laboratory tests the patient will be sent to Quest Lab. Some types of in house labs tests offered are:
• Urine Pregnancy tests
• Hemoglobin
• Random Blood Sugar
• Urinalysis
